DESCRIPTION OF THE PROVISION OF ADMINISTRATIVE SERVICE FOR THE PERMIT ISSUANCE FOR COMMERCIAL FISHING IN THE COSTAL WATERS OF THE BALTIC SEA

4.	Information and documents to be received by the institution (an employee, dealing with the application)	4.1. Information on the fishing quota allocated to the user of the fish stocks for the species (s) covered by the application and, if the fishing quota of this fish species is not allocated to the users of the fish stocks, whether they are included in the lists of the users of the fish stocks, authorized to fish in a certain fishing area, and the lists are approved by order of Director of the Fisheries Service. 4.2. Information on whether an administrative penalty has been imposed on the user of the fishery resource – deprivation of the right to fishing. 4.3. Information on whether the fishing license validity issued by the Republic of Lithuan to the fishing vessel for which the fishing permit is requested, has not been suspended or cancelled. 4.4. Information on whether the fishing vessel of the Republic of Lithuania, for the operation of which a permit is requested, is not included in the EU vessel list for ilegal, unreported and unregulated fishing

9.	Application form, example of filling in and application content	The application is written in a free form. It shall include: 9.1. the type of the fishing permit applied for; 9.2. the name of the fishing vessel for which the fishing permit is being requested; 9.3. the fishing area; 9.4. the species (s) for which the fishing permit is requested; 9.5. the preferred date of entry into force of the fishing permit.
