| 9. | Application form, example of filling in and application content | The application of a natural person shall include: name, surname, the date of birth, the address and signature of the natural person.
The application of a legal entity shall include: the code of the legal entity, the name of company, the address of the registered office, the name and signature of the person entitled to act on behalf of the legal entity. The application is written in a free form. |
