DESCRIPTION OF THE PROVISION OF THE ADMINISTRATIVE SERVICE FOR THE INVESTIGATION OF NOTIFICATIONS, COMPLAINTS AND APPLICATIONS

3.2. A statement by a state politician, an officer or a civil servant.

3.3. Another person’s notification.

3.4. Documents, proving the fact of infringement of rights and legitimate interests (if not available at the Fisheries service).

3.5. A document, proving representation or a copy thereof, in the case of application on behalf of the person represented.

7. Duration of the provision of the administrative service Requests by persons, with the exception of requests from persons which, without prejudice to the interests of the person making the request, of other persons or the institution, can be answered promptly, shall be dealt with within 20 business days from the date of registration of the request within the Fisheries service.
8. Price of the administrative service (if the service is provided not free of charge) Free of charge.
9. Application form, example of filling in and application content A complaint, notification or request shall state the name, place of residence (if a natural person is applying) or other contact details for the response; the name of the entity, registration address (if a legal entity is applying), or other contact information for the response to be forwarded.

When a representative is applying on behalf of a represented person, the request shall include the name, address or other contact details of the representative, as well as the name of the person represented (in the case of a natural person) or the name of entity (in the case of a legal entity applying).

The content shall be in a free form.
