DESCRIPTION OF THE PROVISION OF THE ADMINISTRATIVE SERVICE FOR ARCHIVE DATA ISSUANCE
|1.||Title of the administrative service||Archive data issuance.|
|2.||Legislation governing the provision of the administrative service||2.1. Republic of Lithuania Law on Public Administration.
2.2. Rules of examination of persons’ applications and their service in public administration institutions, enterprises and other public administration subjects” approved by the Resolution No. 875 of the Government of the Republic of Lithuania of 22 August 2007 on approval of the rules of examination of persons’ applications and their service in public administration institutions.
|3.||Information and documents to be provided by a person||3.1. An application (an application shall be signed, the name, surname, place of residence shall be indicated (if a natural person is applying) or the name of the entity, code, head quarter’s address (if a legal entity is applying).
3.2. Identity document or a copy of it if the application is sent by post.
3.3. Documents proving the representation and the identity of the representative, or copies thereof, if the application is sent by post (when a representative is applying for the information about the person represented).
3.4. Depending on the content of the application, relevant information and / or documents or copies thereof (a social security certificate, marriage certificate (if the surname has changed).
|4.||Information and documents to be received by the institution (an employee, dealing with the application)||Information upon request from the Office of the senior archivist of Lithuania and its subordinate archives. (As needed).|

|7.||Duration of the provision of the administrative service||Requests by persons, with the exception of requests from persons which, without prejudice to the interests of the person making the request, of other persons or the institution, can be answered promptly, shall be dealt with within 20 business days from the date of registration of the request within the Fisheries service.|
|8.||Price of the administrative service (if the service is provided not free of charge)||Free of charge.|
|9.||Application form, example of filling in and application content||The application shall be written in a free form. It shall include the person’s name and surname, the place of residence or other contact details for the reply to be forwarded.|
|10.||Sequence chart of the necessary actions to be taken in providing the administrative service||Sequence chart.|
